High blood pressure is one of the most common health problems in adulthood. That is exactly where its danger lies: it often goes unnoticed for a long time, even though over time it can damage the heart, brain, kidneys and blood vessels. Many people feel completely healthy at first. That makes it all the more important to know your own blood pressure readings and be able to interpret them.

Why elevated blood pressure is a problem

For blood to reach every organ, the body needs a certain amount of pressure in the vessels. However, if this pressure stays too high for a long time, the vessel walls are strained over the years. This increases the risk of serious secondary conditions such as stroke, heart attack, heart failure and kidney damage. The eyes can be affected too.

So the real problem is not just a reading on paper, but the long-term strain on vital organs.

Why high blood pressure is often detected late

Elevated blood pressure usually causes no clear symptoms. Some people report headaches, inner restlessness, dizziness, palpitations or reduced stamina. Such symptoms are non-specific, however, and are often attributed to stress, lack of sleep or everyday life. As a result, high blood pressure frequently remains undetected for years.

Regular checks are therefore crucial, especially in middle and older age.

Which readings are considered normal

Blood pressure is given as two numbers. The upper value describes the pressure when the heart pumps blood into the circulation. The lower value shows the pressure during the phase in which the heart fills with blood again.

For healthy adults, values around 120/70 mmHg are considered favorable. High blood pressure is generally diagnosed at a sustained 140/90 mmHg or higher. Different thresholds apply depending on the measurement method. Readings taken at the doctor’s office, home measurements and 24-hour measurements are each assessed slightly differently.

As a rough guide:

  • optimal: below 120/70 mmHg
  • elevated: 120–139 systolic or 70–89 diastolic
  • grade 1: 140–159 / 90–99 mmHg
  • grade 2: 160–179 / 100–109 mmHg
  • grade 3: from 180 / 110 mmHg
  • isolated systolic: upper value elevated, lower value below 90 mmHg

What matters here is this: it is not a single measurement that decides, but the trend across several measurements and days.

Why blood pressure is not always the same

Blood pressure changes quite naturally over the course of the day. Physical activity, excitement, pain, temperature and emotional strain can all raise it. At rest it usually drops again. At night it normally falls as well, before rising again in the morning hours.

Such fluctuations are normal. What matters is whether the readings stay elevated over the long term.

How blood pressure is regulated in the body

Blood pressure arises from the interplay of the heart, blood vessels, nervous system and hormones.

  • The heart determines, through its pumping force, how strongly blood enters the circulation.
  • The vessels influence resistance: narrow vessels raise the pressure, wide vessels lower it.
  • The autonomic nervous system continuously adjusts heart rate and vessel tension.
  • Hormones also play a part by controlling the fluid balance and vascular tone.

These processes run unconsciously and automatically.

How high blood pressure develops

In most cases, no single cause can be identified. This is then called primary or essential hypertension. Usually several factors act together, such as genetic predisposition, increasing age and unfavorable lifestyle habits.

The most important factors you can influence include:

  • being overweight
  • too little exercise
  • smoking
  • high alcohol consumption
  • a very salt-rich diet
  • persistent stress

In addition, there is secondary high blood pressure. Here the elevated blood pressure is the result of another condition or external influences. Possible causes include kidney disease, hormonal disorders or sleep apnea. Certain medications can also raise the readings.

Who should be especially attentive

With increasing age, the likelihood of high blood pressure rises significantly. Men are often affected from older middle age onwards; in women the risk increases especially after menopause. Overall, a large proportion of the older population has elevated readings.

People with a family history, excess weight, diabetes, kidney disease or a lack of exercise in particular should check their readings regularly.

How the diagnosis is confirmed

Before treatment is started, it should first be clarified whether blood pressure is in fact persistently elevated. Single measurements are usually not enough for this. Doctors rely on repeated measurements, home measurement logs or a 24-hour long-term measurement.

Further assessment often includes:

  • a conversation about symptoms, previous conditions and medications
  • a physical examination
  • blood and urine tests
  • an ECG
  • depending on the situation, further examinations of the heart, kidneys or vessels

This is not only about the blood pressure itself, but also about possible causes and any existing organ damage.

What the treatment aims to achieve

Therapy is not meant merely to improve numbers, but above all to reduce the risk of serious secondary conditions. Which target values make sense depends on age, accompanying conditions, general resilience and possible side effects.

Values below 140/90 mmHg are often the goal. For many patients, lower target ranges are also sensible, as long as the treatment is well tolerated.

What those affected can do themselves

Lifestyle changes are a central part of the treatment. Even comparatively small changes can have a favorable effect on blood pressure. These include:

  • losing weight if overweight
  • regular exercise
  • salt-conscious eating
  • moderate alcohol consumption
  • stopping smoking
  • reducing stress
  • good sleep

If these measures are not enough, or if the risk is already elevated, medication is used in addition.
