Writing your readings down is the simplest way to see your blood pressure over time and to show your doctor more than a single number. This free log sheet fits on one page, and you can print it as often as you like.
What is on the sheet
- 26 rows for date and time — about two weeks of morning and evening readings
- Two readings per session: systolic (SYS), diastolic (DIA) and pulse
- A notes column for medication, symptoms or anything unusual
- Fields for your name, the period and your doctor
- The US blood pressure categories and a short measuring guide
How to fill it in
Measure at the same times every day, for example in the morning and in the evening. For 30 minutes before, avoid caffeine, smoking and exercise, then rest for 5 minutes. Sit upright with your feet flat and your bare upper arm resting at heart level.
Take two readings one minute apart and write both down. A single high reading is no reason to panic: measure again and look at the trend over several days.
What the numbers mean
The sheet lists the US categories: normal is below 120/80 mmHg, and from 130/80 mmHg it counts as high blood pressure. Our article on normal blood pressure by age explains why these limits apply to every adult.
When a reading is very high
If a reading is higher than 180/120 mmHg, wait one minute and measure again. If it stays that high, contact your doctor right away. If you also have symptoms such as chest pain, shortness of breath, back pain, numbness, weakness, a change in vision or difficulty speaking, call 911.
Bring it to your appointment
Take the sheet to your next appointment. Readings over several days give your doctor a much more complete picture than one measurement at the practice.
Easier with the app
If you prefer your phone, Beat It! does the same digitally: you enter both values and your pulse in seconds, see 7-day and 30-day trends and export a PDF report for your doctor.
